Scope CONTRACTOR shall administer the 2018 School Health Profiles Survey (Profiles) to a stratified sample of administrators and lead health educators in accordance with Westat (statistical research contractor) and the Centers for Disease Control and Prevention (CDC) procedures and protocol to achieve statewide, weighted data. CONTRACTOR will coordinate project activities with the STATE data manager. STATE will furnish the contractor with all necessary information and data pertinent to the execution of the contract. CONTRACTOR must provide the necessary equipment to successfully complete the project. Specific, Results Oriented Tasks and Deliverables CONTRACTOR will work with the STATEs data manager, CDC and Westat to administer the survey. Tasks to be accomplished include: 1. Coordinate project activities with the STATE. Provide technical assistance and maintain continual communication with STATE to ensure the survey is proceeding in the approved manner until completion of “Profiles 2018.” 2. Provide the STATE a monthly electronic (e-mail format) progress report. 3. Confirm survey plan and timeline deadlines after receiving approval from the STATE. Survey plan and timeline may be adjusted only with prior approval from the STATE. 4. Collaborate with the STATEs data manager, STATEs Management Information Systems (MIS) unit, Westat and CDC in updating the 2018 Sampling Frame for school selection by Westat. 5. Coordinate with Westat to establish a statewide sample of administrators and lead health teachers in randomly selected schools. STATE provides a list of all North Dakota schools and Westat is responsible for randomly selecting the schools from the list. The administration procedures and protocol will be sent to the selected vendor in a three ring binder, titled “Handbook for Conducting School Health Profiles.” 6. Set up a tracking system and/or mechanism for monitoring the return of questionnaire booklets. 7. Order scannable, camera ready, questionnaire booklets provided by Westat. 8. Contact selected schools to enlist their participation in the survey. 9. Prepare written instructions for all survey participants, with final approval by the STATE. 10. Assemble survey materials into packets for mailing and mail to schools and or/survey administrators by January 29, 2018 with the survey return deadline of February 28, 2018. (Note: Questionnaires were mailed to 186 ND schools for the 2016 Profiles, 185 ND schools for the 2014 Profiles; and 236 ND schools for the 2012 Profiles.) 11. Administer surveys according to prepared schedule. 12. Provide unlimited technical assistance to Profiles 2018 survey respondents (schools/districts/STATE) either via phone or email. 13. Implement tracking system and follow-up with non-responding schools. 14. Review incoming questionnaires, assemble documentation forms, sampling information, a copy of the questionnaire and complete answer sheets and mail to Westat for scanning by May 25, 2018. 15. Contractor MUST obtain weighted data for the CDC selected sample by May 25, 2018. If weighted data is not obtained, 10% will be deducted from final payment to CONTRACTOR. 16. Review final report document draft from Westat for accuracy purposes. 17. Generate final progress report for STATE, including advice or recommendations for future Profiles cycles in obtaining weighted data.
