Description:

SCOPE OF WORK:

The PAQ shall only be used to address critical and major problems. The definitions for “Critical” and “Major” are listed below. New software development efforts, or any other work, is NOT permitted via this PAQ and will not be compensated.

Critical Problems: Software errors that require an immediate resolution because they render an application in operable prevent all useful work from being done, disable major functions of the application, significantly impact functions of the business, have no workaround, and/or impact the health or welfare of one or more clients.
• Critical Problems
o Work will commence within 30 minutes of assignment and must be resolved within 48 hours unless a documented reason is supplied.
o No non-critical work will be addressed by the assignee until the critical issue is resolved.

Major Problems: Software errors that require a prompt resolution, significantly impact major functions of the application, have a workaround available, impact the clients eligibility or ability to receive benefits, or result from data integrity problems.
• Major Problems
o Work will commence within 48 hours of assignment.
o At a minimum, 50% of major problems assigned must be resolved within five (5) business days.
o Of those that exceed the five (5) business day turnaround, a minimum of 25% must be resolved within ten (10) business days.
o All remaining new major problem reports must be resolved within twenty (20) business days.

PROJECT APPROACH:

This Critical and Major Support PAQ is being requested under the Statewide Information Technology (IT) Consulting Services contract. This PAQ shall not exceed $753,920.

The contractor will report to the ITSD/DSS Project Manager and will provide contract staff with sufficient knowledge and skill to resolve FACES production problems that are classified as either “Critical” or “Major” using the definitions below. The ITSD/DSS Project Manager in coordination with FACES management will authorize all issue/problem work to be performed and an estimate of level of effort in hours will be provided by the contractor. The ITSD/DSS Project Manager or his designee will make the appropriate resource assignments to complete the work. Work on these problems will commence within a designated time frame based on the category. Each deliverable/problem report assigned under this PAQ will require a description of the problem, the resolution to the problem, the agreed upon level of effort in hours estimate, and the actual time to resolution.

Consultants may be exchanged under this PAQ without modification of the PAQ upon agreement of both parties.
• The state reserves the right to review resumes and accept/reject proposed consultants.
Status reports will be submitted weekly and upon request to the ITSD-DSS Project Manager, or a designee. Status reports will include but are not limited to: key activities conducted during the given reporting period, key activities planned for the next reporting period, and documentation regarding any outstanding issues. See attached ITSD Status Report template.

The State shall have a ten (10) business days acceptance period upon notification of the completion of work/deliverable.
• Additional time for acceptance may be negotiated. Any extension of the acceptance period must be agreed to by all parties in writing, except when deliverables are returned for correction.
• When deliverables are returned for correction, a new ten (10) business days acceptance period will begin upon notification the correction has been completed.
