The automated web-based software will be used to compile information to demonstrate compliance with the federal reporting requirements for the 1915(c) home and community based waiver for individuals with intellectual/developmental disabilities and State requirements of licensing regional human service centers. Implementation of this software will assist State with identifying training needs, policy clarification, and corrective actions. The web-based application must contain the following functionality: Data Entry: • Enable simultaneous gathering and compilation that can be conducted on site or remotely. • Capture information through elements that assess compliance with federal and state regulations. This includes the ability for user to identify which authority (federal or state requirement) the information is being entered for. • Allow State the ability to maintain the users and provider information. • Customize the performance measures or State requirements when federal or state regulations change. Reports: • Provide automated reporting of all data which is designed to monitor and report progress on federal and state requirements. This includes already developed standardized reports and ad hoc reports that can be created by general users. • The following standardized reports must be available: o A summary report that identifies the specific elements, the number of times the element was sited in reviews, total number of cases reviewed, and the total number of incorrect elements cited. o A waiver assurance report that identifies each waiver assurance and performance measure, total reviews completed for each measure, and the total number of incorrect/correct and percentage of incorrect/correct. o A report that allows the user to select a specific element, the review number, consumer name, and the comments from the review. o A summary report that identifies a specific element, the total number of cases reviewed by a specific staff, the number of cases in which the element was incorrect/correct, and the percentage of cases which are incorrect/correct. o A report that provides detail of a review, including: participant name, response due date, past due reviews, reviews that have been completed or are awaiting a response.   Call 502-379-4456 Ext 100 for more details.
