Client: THE CITY OF WATERBURY FIRE DEPARTMENT

Description:

Eligible proposers will be those consultants, businesses, and institutions that have the
following qualifications:

1. Must be an EMS provider / instructor licensed by the State of
Connecticut to deliver continuing education services as designated by the
Connecticut Department of Public Health Office of Emergency Medical
Services (CT DPH OEMS).
2. Must not have any "Licensure Actions or Pending Charges" as listed
by the CT Department of Public Health.
C. Scope of Services
1. Eligible proposers will design and implement an in-service
schedule for the Waterbury Fire Department to provide
recertification of Emergency Medical Responder (EMR) and
Emergency Medical Technician (EMT) to service fire department
members working rotating schedules.
2. Proposers shall be responsible for the initial filing of the T-4 per
CT DPH OEMS.
3. Eligible proposers must be able to deliver services within the
geographic boundaries of the City ofWaterbury.
4. Eligible proposers shall provide a clean classroom with heating
and air conditioning.
5. Eligible proposers shall be able to provide course instruction at a
minimum of three deliveries (09:00-11:00, 11:30-13:30, and 14:00-
16:00) one day between, the hours of 8a.m. -4p.m., weekly.
Courses run on a 9 week cycle. Attached is "Waterbury Fire EMS
Recertification 8/1/22 thru 8/2/24" schedule for reference.
6. Proposals shall meet the minimum standards currently prescribed
by CT DPH OEMS and NCCR requirements for EMR and EMT.
7. Proposals shall include the delivery of all necessary skills
verification per CT DPH OEMS including proctors, equipment,
and submission-of the T-6 form.
8. Proposals shall include CPR recertification including provision of
an eCard per the American HeartAssociation.
9. Proposers must include on-site parking to accommodate up to 4
firefighting apparatus and 4 staff vehicles.
10.Proposals using distributive models shall include
recommendations using the Departments "Lexipol Fire Rescue
1" platform
